Emsculpt vs Emsculpt NEO: What Is the Difference?

Emsculpt vs Emsculpt NEO comes down to one added technology. Emsculpt uses electromagnetic energy to contract and strengthen muscle. Emsculpt NEO uses the same energy and adds radiofrequency heat to break down fat. In FDA terms, Emsculpt is cleared for toning and strengthening muscle, and Emsculpt NEO is also cleared for non-invasive fat reduction in specific areas.

This guide compares how each works, what each is cleared for, what the published studies show, and who each one suits.

How Emsculpt works

Emsculpt delivers focused electromagnetic energy through an applicator placed over the muscle. This triggers strong, rapid muscle contractions. The FDA classifies it as a powered muscle stimulator for muscle conditioning.

Its clearance covers improving abdominal tone and strengthening the abdominal muscles, and strengthening, toning and firming the buttocks, thighs, calves and arms. Fat reduction is not among the uses the FDA cleared it for. Our Emsculpt page explains how treatment is carried out.

How Emsculpt NEO works

Emsculpt NEO combines the same electromagnetic energy with radiofrequency, which heats the fat layer during the session. The manufacturer describes the sensation as intense muscle contractions with a heating feeling “comparable to a hot stone massage.”

The FDA first cleared it in December 2019 for non-invasive breakdown of fat in the abdomen and reduction in abdominal circumference. The manufacturer’s current indications add the thighs, the flanks and the upper arms, with limits by skin type for some areas and a body mass index of 30 or under for the upper arms. What “FDA-cleared” means

Both devices are cleared, not approved. Clearance through the FDA’s 510(k) process means the agency found the device “substantially equivalent” to one already on the market for the stated uses. Clearance is not the same as FDA approval, and it applies only to the uses listed.

What the studies show

A 2024 systematic review in Aesthetic Plastic Surgery gathered the published clinical studies of both devices. It found 15, of which 11 studied Emsculpt and 2 studied Emsculpt NEO.

  • Typical protocol. Four treatments over two to four weeks.
  • Abdomen results. On imaging, fat thickness fell by an average of 5.5 mm and muscle thickness rose by 2 mm.
  • Safety. No complications were reported.
  • Limits. Two studies reported marginal or no benefit. No study reported weight change. The reviewers noted gaps in how some studies reported their participants and data.

One longer follow-up exists for the original device. A study of 21 patients scanned about a year after treatment found that the changes had been maintained, with fat about 15% lower and muscle about 19% thicker than at baseline.

The evidence for Emsculpt NEO specifically is thinner. In the FDA clearance study, participants had a body mass index of 35 or under, and the main result was a reduction in waist measurement at three months. The manufacturer quotes 30% fat reduction and 25% muscle growth, marked as its own data on file. We could not verify those figures from an independent source, so we give them as the manufacturer’s claim.

Which is better for fat, and which for muscle?

If your goal is muscle tone and strength alone, that is what the original Emsculpt is cleared for. If you also want to reduce fat in the treated area, only Emsculpt NEO has FDA clearance for that.

Neither is a weight-loss treatment. The studies measured changes in fat and muscle thickness in the treated area, and none reported weight change. The manufacturer says Emsculpt NEO is for patients with a body mass index of up to 35.

Sessions, downtime and how long results last

The manufacturer’s protocol for Emsculpt NEO is four 30-minute sessions scheduled 5 to 10 days apart, and it states there is no downtime. The studies of Emsculpt used a similar four-session course.

On how long results last, the one-year study above is the longest follow-up we found. Its authors described the finding as needing to be verified by further studies. Ask your provider what maintenance they suggest.

Who should check before treatment

Both devices have contraindications set out in their safety information. Tell your provider about your full medical history, including any implanted devices, pregnancy and recent surgery, so they can confirm whether treatment is suitable for you.

  • Is my main goal muscle tone, fat reduction, or both?
  • Is the area I want treated one the device is cleared for?
  • How many sessions do you recommend, and why?
  • What results should I realistically expect, and when?
  • Is there any reason I should not have this treatment?

Frequently asked questions

Is Emsculpt NEO better than Emsculpt?

It does more, because it adds fat reduction to muscle strengthening. Whether that is better depends on your goal.

Does Emsculpt reduce fat?

Studies of the original device have measured reductions in fat thickness, but its FDA clearance is for muscle toning and strengthening, not fat reduction.

How many sessions do I need?

Four is the usual course for both. Your provider may suggest a different plan.

Do they hurt?
